Happy Faces, which has been running for more than seven years, will be taken over by Prudhoe Youth Project (PYP) this month.
The registered charity offers support to neurodiverse young people through youth club-style sessions, family trips, and activities including bowling nights and cinema trips.
Happy Faces was set up by a group of parents who wanted more opportunities for their children to socialise.
With their children now grown, the parents have decided to pass the responsibility of the charity on to PYP.
Amanda Carr, chairwoman of Happy Faces, said: “As a team we have had an amazing journey over the past seven years, it has been so rewarding and we have made some amazing memories.
“We have worked closely with PYP over the last few years and trust they will continue to support the Happy Faces young people in the way we always have with the added benefits of the wider PYP family.”
Prudhoe Youth Project, which was set up in 2018, has grown over the past four years.
It now has 10 members of staff and runs 11 youth sessions a week.
Gemma Nixon, Youth Project Manager, said: “We have been working with Happy Faces for several years now, so the transition will be a smooth one.
“The importance for us is to ensure this provision is available for SEN young people for many years to come.
“The staff team at Happy Faces are employed by PYP therefore day to day running will remain the same.
“We look forward to using this opportunity to widen our provision to SEN young people by offering additional projects, trips and residential trips.”
